Remember: CMH requires you to mail the original copy of the Waiver with all signatures. Faxes, Scans and Emails are not acceptable.

Open and Print the waiver in the language of your choice.
Before printing, set Page Scaling to "Shrink to Printable Area".
Complete the form in pen.
Both applicant and witness must sign as indicated.
Initial each page of the document.
Mail the completed form to CMH (see address at right).
REMINDER: We cannot accept scanned, faxed or email copies.
