CMH Summer Adventure Waivers

Lose your original Waiver? Make a mistake and need a new copy? Whatever the need, CMH Heli-Hiking offers 2 options, making it easy to complete your Heli-Hiking Application and Waiver.

Remember: CMH requires you to mail the original copy of the Waiver with all signatures. Faxes, Scans and Emails are not acceptable. Be sure to read the document carefully and ensure it is properly filled out and initialed.

Option 1 - Digital Waiver (English only)

• Download the Digital Waiver.
• Use a standard PDF program to complete in the form digitally.
• Print the form. (Note - you cannot save the completed form)
• Both applicant and witness must sign and initial as indicated.
• Mail the completed form to CMH (see address at right).

Option 2 - Print Waiver (English, Japanese)

• Download the Print Waiver.
• Print the form.
• Complete the form in pen.
• Both applicant and witness must sign and initial as indicated.
• Mail the completed form to CMH (see address at right).
